Documentation of Practice Situation (UCHWU)
As UCHealth Workers United (UCHWU), we are advocating for improved staffing and caring conditions. Frontline health care providers, under the laws of Colorado, are responsible and accountable to our patients and to uphold the standards of our licensure. If you have a work assignment which you believe is unsafe and places your patients or you at risk, as always follow your standard workplace protocols for reporting (RLs, etc). This form is for UCHWU collective documentation of practice situations which we believe are unsafe. No fields are required; complete on non-work time. Note: exclude HIPAA protected information.
